As you may know, the Kentucky Conference on Health Communication is funded in part by the National Institutes of Health, specifically the National Cancer Institute and the Office of Behavioral and Social Sciences Research. To assist us in preparing reports on KCHC to NIH, we would like to ask you to voluntarily report race/ethnicity and gender information with your conference registration. We understand such reporting is a complex and sensitive issue, and we respect your right to skip these questions if you prefer. We include the categories that NIH uses; we also provice you with the option to describe yourself using an open-ended response if you prefer. Pre-Conference Luncheon - Thursday, April 12, 2018

All entrees include iced tea, Torrefazione Italia® coffee, a selection of Tazo Tea®, and chef's choice of dessert. Please choose one of the following options:

Bluegrass Club- Croissant Stacked with Roasted Turkey, Smoked Bacon, White Cheddar Cheese, Seasoned Mayo, Lettuce, and Tomato; served with House Made Chips and Chef's Selection of Prepared Salad

Traditional Chicken Caesar Salad- Parmesan-dusted Grilled Chicken Strips atop Fresh Romaine, blended with Grated Italian Cheeses and Garlic Croutons, served with Caesar Dressing

Vegetarian Wrap- Shredded Romaine and Iceberg Lettuce with Julienne Yellow Squash, Zucchini, Carrots, Red Peppers and Asparagus Drizzled with Chef's Choice of Dressing and Wrapped in a Flour Tortilla; served with Chips and Chef's Selection of Prepared Salad (vegetarian selection)
